Asbestos Exposure Linked to Genetic Mutation
Newswise — Mice inhabiting a northern town of Israel known for its high concentration of asbestos-contaminated dust, have a higher level of genetic somatic mutations, compared with other regions where asbestos pollution levels are lower. This has been shown in a new study carried out by Dr. Rachel Ben-Shlomo and Dr. Uri Shanas of the University of Haifa's Department of Biology in Oranim. "This study clearly indicates that there is a link between the higher levels of asbestos in the environment and the frequency of genetic somatic mutations in the mammals," the scientists said.
Earlier studies of asbestos have already shown that the thin fibers, which penetrate the body by inhalation or through consumption of food contaminated with the material, not only cause certain cancers but also genetic mutations in DNA structures. It is also known that asbestos is a material that decomposes slowly, over many years. Data from the Israeli Ministry of Health indicate a rise in the number of cancer patients from exposure to asbestos in the Western Galilee region of northern Israel, and therefore the scientists set out to examine whether genetic mutations are found in the mouse population living in its northern town of Nahariya. They chose to probe mutations in mice because their generations are renewed every three months, so it could be assumed for the study that dozens of generations of this sample population in Nahariya had already been exposed to the fibers.
Wild mice from two locations were sampled – one group living close to a factory that manufactured asbestos-based products in Nahariya during 1952-1997, and a second group from a town located 50 kilometers, or 31 miles, from Nahariya and where no known asbestos pollutants are found. Samples were taken from both groups and six sites in the DNA were examined for genetic differences between the groups.
The results indicated differences between the groups' DNA and that the Nahariya-based mice had higher levels of genetic somatic mutations.
"These findings teach us that the pollutive, mutagenic asbestos increases somatic mutational frequency, which can in turn heighten the chances of developing cancerous growths," the researchers concluded.

Combination Therapy Shows Promise for Rare, Deadly Cancer Caused by Asbestos

Newswise — Philadelphia – Pleural mesothelioma patients who undergo lung-sparing surgery in combination with photodynamic therapy (PDT) show superior overall survival than patient treated using the conventional therapy of extrapleural pneumonectomy (EPP) (or en bloc removal of the lung and surrounding tissue) with PDT, indicates new research from the Raymond and Ruth Perelman School of Medicine at the University of Pennsylvania. The research is published in the June 2011 issue of the Annals of Thoracic Surgery.
“Unlike patients who receive traditional lung sacrificing surgery for mesothelioma, the patients in our study who underwent lung sparing surgery and photodynamic therapy, a light-based cancer treatment, have experienced unusually long overall survival rates. The median survival for those patients had not been reached at over two years when the results were analyzed. That’s unusual in this field, especially when the majority of those patients are older and have advanced cancer,” said Joseph Friedberg, MD, co-director of the Penn Mesothelioma and Pleural Program and the thoracic surgeon who performed the operations cited in the study. “In addition to the overall survival statistics, the difference between having and not having a lung, both with respect to the risk of surgery and the ability to enjoy a normal life after surgery, is crucial for these patients.”
Mesothelioma is one of the most aggressive and deadliest forms of cancer and is usually caused by exposure to asbestos. Exposure to asbestos typically precedes development of the cancer by anywhere from 10 - 50 years, but once it occurs, the average survival rate following diagnosis is often only 9-12 months.
Although mesothelioma can occur in other locations like the abdomen, pleural mesothelioma is the most common form of the disease and accounts for roughly 70 percent of cases. This form originates in the pleura – the membrane surrounding the lung and lining the chest – where it starts off as a microscopic sheet of malignant cells that coats the interior of the chest and can grow to be several inches thick. The coating and enveloping nature of the cancer makes it impossible to completely remove it with surgery alone. As a result, the conventional surgery-based approach to treatment involves radical surgery that includes removing the lung, in combination with chemotherapy and whole chest radiation. Even with this aggressive treatment, the disease will recur in almost all patients.
The current study had two goals. The first was to determine if using a new combination of PDT and surgery would allow a less extensive surgical procedure to be used in lieu of an EPP. The second was to determine if, based on previous research from Penn with PDT, the treatment would have any positive effect on survival for patients.
Unlike radiation, which passes through the body, the PDT therapy used in the current study penetrates only a short distance which allows the lung to be preserved. The PDT treatment aims to eradicate the remaining microscopic disease trigger a patient’s own immune system to help fight cancer. Penn is one of only two centers in the world where PDT is used to treat pleural mesothelioma.
In the study, 28 patients (19 men, 9 women) underwent surgical resection plus PDT for pleural mesothelioma. Patients were aged from 27 to 81 years. All patients were seen in a multidisciplinary setting and educated about the spectrum of treatment options available, including surgical intervention and its currently investigational status. Of the study group, 14 patients were treated by modified extrapleural pneumonectomy (MEPP) and 14 by radical pleurectomy (RP) and intraoperative PDT. Twenty-two of the 28 patients also received chemotherapy.
Demographics in the MEPP and RP cohorts were similar in age, sex, stage, nodal status, histology, and adjuvant treatments. Stage III/IV disease was present in 12 of 14 patients (86 percent) in both groups. The median overall survival for the patients who received the MEPP treatment was 8.4 months. At a follow-up 2.1 years after the end of treatment, a median survival rate for the patients who received a radical pleurectomy had not yet been reached. The results yielded by the radical pleurectomy and adjuvant PDT were superior to other studies of surgical treatment plans with patients of similar demographics.
“Our primary motivation in attempting the lung sparing surgery was preservation of quality of life, and we were hoping the survival results would at least be similar to the more traditional pneumonectomy approach,” said Friedberg. “Although our pneumonectomy results were in line with what is often reported for similar patients having surgery-based treatments, we were completely caught off guard when the analysis revealed a significantly longer survival for the patients who retained both lungs.”
Although all patients in the current study received the PDT therapy in combination with a different surgical technique, the researchers further note that the use of intraoperative PDT is the evident difference between the multimodal protocol used in the current study and other standard treatment options presently in use, and it seems worth speculating on any potential direct contributions of the PDT to the overall survival rates.
“Why this is happening is unclear and has emerged as the focus of our continuing research,” said Friedberg. The possibility exists that the residual PDT-treated microscopic disease induced an autologous tumor vaccine effect or potentially enhanced the effect of adjuvant treatments.
“This study has limitations, as many mesothelioma studies do, but these results are very encouraging. The findings from our study are particularly notable because many of the patients in this study would often be excluded from surgery-based therapy because of their advanced age or unfavorable oncologic characteristics such as the large bulk of their cancers or the spread to the lymph nodes,” said Dr. Friedberg. “Based on these results this lung-sparing technique, combined with photodynamic therapy, has become the backbone of our surgery-based treatment protocols.”
A larger study investigating the efficacy of this multimodal approach is currently underway at Penn.

Law firm Simpson Millar LLP is urging hundreds of people who could be owed £5,000 each for an asbestos-related condition to get in touch before time runs out.
After the Government set up a fund to compensate people with the condition Pleural Plaques last year, Simpson Millar began providing free services to help claimants navigate their way through the application process, gathering medical evidence and filling in forms. So far, it has secured £320,000 between 64 people.
However, the fund will close its doors to applications in August and Simpson Millar is yet to hear from more than 150 people it contacted who could be entitled to the payment.
Rebecca Ryan, solicitor at Simpson Millar who has been managing the campaign explains: "We have recently written again to many of the people we believe may be entitled to compensation for pleural plaques, informing them of the approaching deadline and I would like to encourage those people to get in touch. It is clear that sometimes it just takes time for people to get around to getting in touch but time is now running out."
Simpson Millar’s lawyers are experienced in handling industrial disease claims and Emma Costin who is head of the firm’s Disease team said it was right to offer claimants some assistance in making their applications to the Pleural Plaques Compensation Scheme on a pro-bono basis.
"All we have asked in return from applicants is that they consider a minimal donation of £10 to the asbestos disease charity Mesothelioma UK," she said, adding that so far the firm has raised almost £700 for the cause to date.
Pleural plaques is a scaring of the lungs caused by the inhalation of asbestos dust. They are very common amongst people who worked with asbestos in Britain’s heavy industries until legislation curbing its use was introduced through the 1960s and 1970s. Unfortunately the substance was still in circulation until the 1980s and beyond as a heat insulating material. A diagnosis of pleural plaques is a clear sign that someone has been exposed to asbestos and as such that they may also be at risk from more serious illnesses such as asbestosis or mesothelioma.

Health Law Expands Medicare To Montana Asbestos Patients

Julie Johnson grew up in Libby, Mont. -- a picturesque Rocky Mountain community that was unknowingly poisoned for decades by deadly asbestos from a vermiculite mine six miles from town.
Her grandfather, a miner, her grandmother and an uncle died of asbestos-related disease. Both of her parents have the condition that has no cure. But she didn't think it would hit her -- until doctors last year found a spot on her lung.

Like many of the nearly 2,000 people from Libby who have asbestos-related disease and the 400 who have died from it, Johnson never worked at the mine, which was owned by chemical manufacturer W.R. Grace. It produced much of the world's supply of vermiculite, a mineral used in insulation, fireproofing and soil conditioning, before closing in 1990.
But tremolite, a form of asbestos that was laced in the vermiculite ore, had been spread throughout the Libby area. Miners brought it home in heavy dust on their clothes helping to contaminate their families, residents took vermiculite to use for insulation and tailings were spread as ground filler around town, including at school and community sports fields. Federal officials in 2009 declared the area the first national public health emergency and have called it the nation's worst environmental disaster.
Without health insurance, Johnson, 45, relied on a state-federal grant program and a medical plan funded by W.R. Grace to help pay for her medications and doctor visits. Yet, with Grace in bankruptcy and the grant program scheduled to end this summer, Johnson was concerned about how she was going to keep up with mounting health bills for medicines and tests.
The 2010 federal health law is providing help. One provision, authored by Senate Finance Chairman Max Baucus, D-Mont., gives full Medicare coverage to people who had lived in Libby for a total of at least six months over a 10-year period before their diagnosis of an asbestos-related disease.
"I feel good knowing I have this [coverage] and it’s never going to go away," said Johnson. She is one of about 600 people from Libby who have signed up for Medicare as a result of the law, according to the Social Security Administration, which oversees the program with the Centers for Medicare and Medicaid Services.
But she and others worry about Republican efforts to repeal the law.
"Losing this benefit would really knock the bottom out of people's lives," said Gayla Benefield, a Libby resident and Johnson's mother.
U.S. Rep. Denny Rehberg, R-Mont., is among those fighting to overturn the law. "It's tragic that assistance for so many deserving folks up in Libby was tied to the anchor of Obamacare, with all of its mangled policy, unconstitutional mandates and reckless deficits," he said.
Rehberg, who has announced he is challenging Democratic Sen. Jon Tester and is making the health law an issue in the campaign, said he will continue working to help the people of Libby but would not say if he supports keeping the Medicare option for them.
Baucus defends the coverage for Libby. "This provision is important because it will provide vital medical services to Americans who — through no fault of their own—have suffered horrible effects from their exposure to deadly poisons," he said.
Expansions Of Medicare Have Been Rare
It is just the third expansion of Medicare eligibility since the federal health insurance program was established for the elderly and disabled in 1965. In 1972, Medicare added people with end-stage kidney disease and in 2001, people with amyotrophic lateral sclerosis, also known as Lou Gehrig’s disease.
Unlike other deals senators secured in the law to help their own states, the Libby provision elicited no public outcry, mostly because of its relatively minor cost. The Congressional Budget Office said it would cost $300 million over the next decade.
For Johnson, being on Medicare means long-term security. Because of the contamination, "W.R. Grace proved that they are not to be trusted and I definitely wasn't banking on having their medical plan for the rest of my life," said Johnson who now lives in Kalispell, about 90 miles east of Libby.
Even with the new Medicare coverage, Grace has no plans to close its Libby medical plan, which has paid out about $21 million since 2000 and has about 1,100 current patients, said William Corcoran, a vice president at Grace. About 70 percent of those in the company plan are 65 and older.
Grace, a chemicals manufacturer based in Columbia, Md., that bought the mine in 1963, has been paying medical bills in Libby for years and agreed in 2008 to spend $250 million for an environmental cleanup of the town. The company, which was driven into bankruptcy protection in 2001 by tens of thousands of asbestos poisoning claims mostly unrelated to Libby, reached a tentative civil settlement in 2008 to pay $3 billion to asbestos victims nationwide.
The federal-state grant program, which serves about 1,400 people, was intended as temporary aid and is slated to end in July.
That program was set up because the Grace medical plan failed to cover everyone who needed help, said Benefield, who also been a leading advocate to get help for the town’s residents. The Grace program hires its own doctors to independently verify if applicants have the type of asbestos-related disease connected to the Libby mine. The government program doesn’t have this independent verification process. Applicants merely have to submit information from their own doctor certifying they had asbestos-related disease.
In 2010, the company accepted 50 of the 83 people who applied for the program. Grace officials said those rejected could not prove their disease was caused by asbestos. For example, if the lung disease was caused by smoking, it would not be covered.
Broader Coverage Under Medicare
Both the Grace and the government grant program pay only for health services related to asbestos disease and both supplement an individual’s private insurance. In contrast, the Medicare coverage pays for all types of health needs.
"The Medicare coverage will be extremely important," said Tanis Hernandez, outreach coordinator of the Center for Asbestos Related Diseases, the government-funded clinic. She said many people have not yet enrolled because they can still get help from the grant program.
Exposure to asbestos is known to cause cancer and other lung diseases. The asbestos-related disease can take decades to show up on X-rays and other tests.
Out of a population of more than 10,000 people in Libby and the surrounding valley, new cases are being diagnosed at a rate of about four a week, according to Hernandez.
"I thought I was fine and I was not a smoker and was least exposed of my siblings," said Johnson who works as a receptionist at a veterinary clinic. But there were early signs of health problems: She caught pneumonia when she was eight months pregnant, was diagnosed with Swine flu, and in the past few years she developed ear infections that took months to clear.
These days, when she shows her Medicare card at the doctor's office, Johnson sometimes gets strange looks. "They say, 'You look so young. How can you be on Medicare?' Then I explain I'm from Libby."

“There is nothing humorous about the devastating impact that asbestos has had on countless workers and their families across the United States and around the world,” said Belluck, a partner of the New York City personal injury law firm, Belluck & Fox, LLP.
“The millions of people who watched this TV program hopefully took away from it an understanding that there are no safe ways to handle or use asbestos, and that it is completely outrageous that this toxic mineral continues to be produced,” he said.
The Jeffrey Mine has received, in principal, a $58 million loan guarantee from the Quebec government that will allow it to expand into underground operations, according to the Montreal Gazette.
The expansion could result in the mine eventually producing up to 225,000 tons of chrysotile asbestos per year, with most of the exports going to India, the newspaper said.
In The Daily Show segment, called “Ored to Death,” Jeffrey Mine Inc. president Bernard Coulombe tells astounded correspondent Aasif Mandvi that chrysotile asbestos is “safe – relatively.”
Later in the segment, Dr. Matthew Stanbrook, deputy editor of the Canadian Medical Association Journal, rejects Coulombe’s assertions about the safety of the mine’s product.
“All forms of asbestos, including chrysotile, cause cancer and other harmful lung effects,” Stanbrook says. “Unquestionably, the mine should shut down.”
Asbestos was once widely used in a variety of products in the U.S., Canada and elsewhere, including insulation, automotive brakes, fireproofing, pipe covering, cement mixtures, joint compounds and floor tiles. Construction and industrial workers have been especially exposed to the mineral.
Although it is still not banned, asbestos use is significantly limited in the United States. However, it is used around the world in developing countries.
“As an attorney who has seen up close the impact of asbestos exposure on the lives of mesothelioma victims and their families, I can’t imagine how anyone would support expanding this mine’s operation,” Belluck said. “It’s time to put an end to asbestos production.”
